Insurance
I am in network with PacificSource, Kaiser, Moda and Aetna insurances.
If you would like to use out-of-network benefits I can provide you "superbills" which you can submit to your insurance company to seek reimbursement. You will be responsible for full payment for sessions, due at the time of your session.
If you plan to seek reimbursement through your insurance, before our first appointment together, please call your insurance coverage and have the following questions answered:
Do I have mental health benefits?
Do I have a deductible and how much, if any, has been met?
How many sessions per calendar year are covered?
Is pre-authorization required? If so, what is my authorization number?
What is my co-pay or co-insurance responsibility?
How much does my plan cover for an out-of-network provider? Will it cover tele-health?
Payment
Payment is due on the day of service.
I can accept cash, check, or credit card.
If you do not plan to use insurance at all, it is important to know that I will not be required to assign you a mental health diagnosis as a part of treatment. Additionally, there will not be a fixed limit on the number of sessions available to you. You will also be able to determine your own goals for therapy. As a private pay client you have the right to receive a “Good Faith Estimate” explaining how much your therapy will cost. Under the law, I will provide you an estimate of the expected charges for our work together. You can ask me for a Good Faith Estimate before you schedule a service or at any time during our work together. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
